Special Accommodations Form for ASD.

Hello,

I was reading up a bit on Asperger's and it might potentially be a factor in my performance speed not being as optimal as I'd like to.
I noticed that to submit a special accommodations form it mentions "such as those covered by the Americans with Disabilities Act" Does Asperger's or autism spectrum disorders count? Would my family doctor be enough for the form? Is there a preferred support channel for me to ask these type of questions?
